Initial Paper Work

Your can fill your initial paper work out on line by going to Patient Forms.

You can also come in 20 minutes early for your appointment and fill out the necessary forms in our office.

Evaluations

When necessary we will perform all necessary tests, evaluations, computerized nerve scans and x-rays. Ask about our payment at time of service discounts. When determined appropriate, treatment can be given on your initial visit.

Report of Findings

We will analyse your test and evaluations and prepare a report to review with you.

We will always give you our best recommendations to get you back on the road to health so you can enjoy your life. We are called a Wellness Center not a sickness center.
